New Clients
If you are interested in receiving services, please fill out a new client inquiry. Once that is filled out and an evaluation has been scheduled, you will be asked to fill out the Intake Paperwork.
The new client inquiry form is located under the “New Clients” tab on the menu.

Insurance Information
We are in-network with Anthem, Blue Cross Blue Shield, Anthem HealthKeepers, Aetna, Optima, United Healthcare, Cigna, Medicare, and Medicaid/CCC Plus (Molina, Va Premier, UHC, Aetna Better Health, Optima, Healthkeepers Plus). We can also provide services for Tricare families.
If we are not in network with your insurance provider, please call for out-of-network payment options.
New Client FAQ
What does Spot On Therapy need from me to begin the scheduling process?
- Completed New Client Inquiry form
- Referral from your primary care physician for Evaluation and Treat (ST/OT/PT) faxed to us
- Insurance Card information – image/or scanned copy front and back
What is your fax number?
Our fax is 1-804-412-8105. Please have your doctor include the 1 in front of the number when they fax over prescriptions.
How long is the waitlist for services?
On average, new clients have a wait time of a couple months for an evaluation and to begin services. Once we have the new client inquiry, insurance information, and referral from your doctor, we will place you on our waitlist. Our front office will notify you when there’s an opening.
Does insurance cover neurofeedback services?
Neurofeedback is not typically covered by insurances in Virginia. Please call for more information.
What is the difference between the New Client Inquiry and New Patient Intake Paperwork?
The New Client Inquiry gives us a general overview of what types of services you are looking for before starting as a client.
The Intake Paperwork is filled out once the evaluation is scheduled and includes all the necessary documents you need to fill out before receiving services. It also allows you to provide any medical and background information that will help the therapist during the evaluation and following treatment. You can find the Intake Paperwork under “Forms.”
